Envisioning an integrated rehabilitation system in northeastern Ontario.
The North East Rehab Network supports the premise that rehabilitation is not just a destination or geographic location within a single organization. Early and effective rehabilitation is a philosophy of care that can and should occur at all points along the care continuum including acute care, long-term care and community based care. When effectively provided by an engaged interprofessional team that includes family members, a rehabilitation focus will improve an individual’s independence and function and can reduce ALC days, hospitalizations and demand for LTC beds. It should be supported by a system that endorses long-term follow-up and successful community re-engagement.
What is the North East Rehab Network?
The North East Rehab Network is comprised of representation from the five northeastern Ontario hospitals with designated rehabilitation beds, the North East Community Care Access Centre (NE CCAC), the Northeastern Ontario (NEO) Stroke and ABI Networks and also includes a representative from the North East Local Health Integration Network (NE LHIN). Our Network was formed in the fall of 2006 and we meet regularly by teleconference. The services of all involved in the Network are in-kind.
Our Purpose
The purpose of the North East Rehab Network is to provide a forum for collaboration, communication, and consensus building to enable Network members to work toward developing a vision of an integrated rehabilitation system in northeastern Ontario.
Our Network strives to:
- Coordinate service and promote equitable access to rehabilitation services for residents of northeastern Ontario. To achieve this we must work cooperatively to improve processes for referring patients for rehabilitation services.
- Address gaps, reduce duplication, and measure overall performance of rehabilitation services in northeastern Ontario.
- Act as a credible advisor to the NE LHIN on rehabilitation practice, policy and funding issues.
- Act as a conduit in connecting with other rehabilitation networks at a provincial level.
